13 Hospitals in the Philadelphia Region and 2 Hospitals in Delaware Earn an “A” in the Fall 2025 Hospital Safety Grades from The Leapfrog Group

 35% of Hospitals in Southeastern Pennsylvania and 42% of Hospitals in Delaware Able to Sustain “A” on the Leapfrog Hospital Safety Grade Over Two Years

Philadelphia, PA, November 24, 2025 – The Leapfrog Group, an independent national watchdog nonprofit, representing hundreds of the nation’s most influential employers and purchasers of health care, announced the fall 2025 Leapfrog Hospital Safety Grades. Driven in Southeastern Pennsylvania and Delaware by the Greater Philadelphia Business Coalition on Health (GPBCH), these grades give the community critical insight about local hospitals and their ability to protect patients from harm.

According to Leapfrog, an estimated 160,000 lives are lost annually from avoidable medical errors, and over 50,000 lives could be saved every year if “B”, ”C”, ”D”, and “F” hospitals improved their patient safety grades to an “A”. In the Philadelphia Region, 13 hospitals and 2 hospitals in Delaware earned an “A,” and Pennsylvania ranked eighth in the country overall for percentage of “A” hospitals. In addition, 35% of hospitals in Southeastern Pennsylvania and 42% of hospitals in Delaware were able to sustain an “A” on the Leapfrog Hospital Safety Grade over the last two years. Hospital Safety Grades are updated twice per year by The Leapfrog Group.

We appreciate the commitment to excellence and the focus on continued improvement in all areas of quality and safety.”

For the fall 2025 Hospital Safety Grades in Southeastern Pennsylvania, 32 hospitals were graded, and 13 earned an “A”, 8 earned a “B”, and 11 earned a “C”. In Delaware, 7 hospitals were graded, 2 earned an “A”, 3 earned a “B”, and 2 earned a “C”.

The Leapfrog Hospital Safety Grade is the only ratings program focused exclusively on accidents, errors, injuries and infections that harm or kill patients in hospitals. It is updated twice a year in the fall and spring, offering the public a reliable, independent assessment of hospital safety. The Grades are determined by public, peer-reviewed methodology, calculated by top patient safety experts under the guidance of a National Expert Panel.
